Care Coordinator

Responsibilities and Duties:

  • Conduct post discharge follow-up via phone call and/ or home visit to ensure patients and caregivers’ ability to cope at home by helping to streamline and coordinate the range of community service available.
  • Implement timely and appropriate care coordination and evaluate the outcome accordingly.
  • Provide a valuable link by ongoing collaboration among the patients, families and the multidisciplinary teams.
  • Monitor patient’s general medical condition during home visit and report to clinician if necessary
  • Promote positive changes in patient’s lifestyle in the community.
  • To help patients and their families to plan for and improve end of life care
  • To participate in hospital activities that contribute towards improving the quality of patient care such as research, quality or innovative projects that enhance patient care
  • Preparation of reports and statistics as required.

Job Requirements:

  • Must possess diploma/ degree or equivalent professional qualifications in Nursing, Social Work or Allied Health or/and any clinical disciplines relevant to the job.
  • At least 3-5 years of experience in nursing or related field in acute and or community settings in Singapore
  • Must be a critical thinker, self-motivated, able to work as part of a multi-disciplined team, and have excellent interpersonal and communication skills.
  • Ability to use local languages and dialects will be an advantage.
  • Equipped with basic computer skills in MS World, Excel, and PowerPoint.
  • Knowledge in geriatric care, statistics, and community services will be an advantage.
