Responsibilities

Coordinate and provide patient care activities using IT systems to track clinical indicators and document care provided.
Assist in closing preventive screening and vaccination gaps related to national health programs.
Perform basic health assessments, including parameter measurements.
Educate patients on lifestyle management through health literacy and social prescribing initiatives.
Foster relationships with internal care teams and external stakeholders, promoting collaboration and support.
Advocate for improved health outcomes by gathering and addressing social determinants of health.

Requirements

Diploma in any discipline
Healthcare experience will be advantageous
Willing to work 5.5 days
Comfortable working in a fast-paced environment
