• Coordinate and provide patient care activities using IT systems to track clinical indicators and document care provided.
• Assist in closing preventive screening and vaccination gaps related to national health programs.
• Perform basic health assessments, including parameter measurements.
• Educate patients on lifestyle management through health literacy and social prescribing initiatives.
• Foster relationships with internal care teams and external stakeholders, promoting collaboration and support.
• Advocate for improved health outcomes by gathering and addressing social determinants of health.

• Diploma in any discipline
• Healthcare experience will be advantageous
• Willing to work 5.5 days
• Comfortable working in a fast-paced environment
