Stroke Support Station (S3) is a Singapore-based non-profit with a big heart and an even bigger mission: to help stroke survivors and their caregivers live fuller, more empowered lives.
We do this through holistic, person-centred programmes that support rehabilitation, build confidence, and strengthen community reintegration. What You'll Do:
Client Care Coordination & Support- Process and schedule new referrals in a timely manner
- Conduct initial client assessments during admission using the BPSS framework
- Conduct reassessments at designated review time points
- Assess clients’ and caregivers’ psychosocial and emotional support needs, and provide basic emotional support, supportive listening, and follow-up sessions where appropriate
- Assist clients/caregivers with appropriate information, referrals, and community resources
- Highlight cases requiring psychosocial intervention during multidisciplinary team (MDT) discussions and collaborate with multidisciplinary teams to implement appropriate interventions
- Conduct joint home visits with Case Managers, or multidisciplinary team members where required
- Conduct means testing for all clients
- Perform financial assessments for clients requiring financial assistance
- Process and monitor financial assistance applications where necessary
- Track and run clients’ change in means test reports twice monthly
- Ensure subsidy-related documentation is accurate and updated
- Prepare and complete service agreements and addendums
- Maintain timely and accurate case documentation and records
- Consolidate, prepare, and track monthly reports and statistics
- Support operational and logistical aspects of programmes and services
- Assist with client and caregiver support events or projects
You Have:
- Minimally a Diploma in Social Work, Psychology, Counselling, Community Care, Gerontology, Healthcare Administration, or related fields
- Relevant experience in healthcare, community care, rehabilitation, eldercare, social service, or case management settings will be an advantage
- Experience and is comfortable interacting with stroke survivors, and caregivers
- The ability to assess psychosocial and emotional support needs with empathy and professionalism
- Experience handling means testing, financial assistance processing, and subsidy tracking
- Strong organisational, documentation, and administrative skills
- Experience managing statistics, reports, and data tracking accurately
- Good communication and interpersonal skills
- The ability to work collaboratively within a multidisciplinary environment
