As a Senior Executive, Primary Care Engagement & Transformation (Healthcare), you will be responsible for the following duties:

• Implement partnership strategies that build the network of primary care and community partners, to provide accessible, appropriate and affordable care in the community.
• Foster and maintain relationships with GPs to understand their strengths and needs, and collaborate to meet the health and care needs of residents in Singapore.
• Manage contact database and strategic alliance with Primary care partners to ensure continuity of partnerships and collaboration.
• Support primary care partnerships for seamless care integration, improved access, quality improvement, and capability development.
• Ensure program and project effectiveness by driving continuous improvement and completing assigned tasks.

Requirements:

• Degree holder in any related discipline.
