• Evaluate residents’ health status and identify risk factors through comprehensive assessments.
• Collaborate with residents and stakeholders to set achievable health goals.
• Develop personalized health coaching plans to support residents in reaching wellness objectives.
• Assist residents in overcoming lifestyle and behavioural challenges to improve overall health.
• Monitor, document, and report residents’ progress and health outcomes.

• Nitec or Diploma or Advanced Diploma in a healthcare-related discipline
• Candidates with a non-healthcare related diploma but with a strong interest and relevant experience in community services are also encouraged to apply.
• Ability to work independently and collaboratively within a team.
• Strong communication and interpersonal skills, especially in engaging older adults.
• Excellent organisational, coordination, and relationship-building abilities.
• Proficiency in computer applications, including Microsoft Office.
• Must be willing to travel and work during weekends if there's any events
