As a Care Coordinator (West) you will be responsible for the following duties:

• Financial & Resource Guidance: Assist families with financial assistance schemes and subsidy applications, ensuring all means-testing renewals are tracked and processed accurately.
• Onboarding & Patient Experience: Manage the end-to-end intake process for new clients, including scheduling initial clinical assessments and resolving inquiries regarding rehabilitation services.
• Collaborative Case Support: Work alongside Social Workers and Clinicians to identify patient needs, providing basic case management and community resource referrals.
• Operations & Outreach: Participate in multi-disciplinary case discussions, conduct home visits when necessary, and support community outreach events or caregiver support projects.
• Data & Compliance: Maintain meticulous records within the clinical management system and prepare monthly statistical reports to track service delivery performance.

Requirements:

• Possess a Diploma or Advanced Diploma (Healthcare Administration, Social Services, or related fields are highly preferred).
