As a Social Work Associate (Healthcare Sector), you will be responsible for the following duties:

• Perform assessment in financial application cases
• Assisting patients in national financial schemes application
• Assist medical social worker in reviewing assessing cases
• Work together with medical social worker in providing brief social assessment
• Collecting and organizing information for case assessment
• Perform home visits when necessary
• Educating patients and family members
• Act as mentors to new Social Work Assistant within the team
• Participate in quality improvement projects when necessary
• Any other duties within the team

Requirements:

• Min. Diploma in Social Work or equivalent.
