Social Work Coordinator [Up to $3,000!]

Responsibilities:

• Provide administrative and operational support to the department, including counter management and related duties
• Conduct patient intake interviews, financial assessments, and provide appropriate assistance to patients and families
• Support discharge planning through referrals in selected areas of care planning
• Assist Medical Social Workers with case management, including translation support, patient escort, and home visit assistance
• Collaborate with internal departments and external organisations to facilitate patient care and service delivery
• Participate in work process improvements and quality improvement initiatives
• Support departmental programmes and projects

Requirements:

• Diploma in Social Work
• Reports to a Medical Social Worker and works closely with the Team Leader, Social Work Coordinators
• Good interpersonal, communication, and problem-solving skills
• Ability to interact effectively with patients, families, and multidisciplinary teams
• IT knowledge and competency in using relevant systems and applications
• Ability to support translation needs is an added advantage for liaising with patients and families who are non-English speaking
