As a Social Work Assistant (Healthcare), you will be responsible for the following duties:
- Perform assessments for patients’ financial assistance applications and support related case evaluations.
- Assist patients in applying for national financial assistance schemes and explain the available support options.
- Support Medical Social Workers in reviewing and assessing patient cases
- Work closely with the Medical Social Worker to provide brief social assessments and case support.
- Collect, organize, and maintain relevant information and documentation for case assessments.
- Conduct home visits when required to better understand patients’ social and living conditions.
- Provide guidance and education to patients and their family members on available support services and resources.
- Act as a mentor to newly joined Social Work Assistants within the team.
- Participate in quality improvement initiatives and support departmental projects when required.
Requirements:
- Minimum Diploma qualification in Social Work or equivalent field.
