Wellness Coach (Healthcare)

Plan, promote, and implement wellness programs and activities at the community centres.
- Conduct screenings, physical assessments, and track patient progress with follow-ups.
- Monitor patient participation and compliance with individualized care plans.
- Maintain and disinfect program equipment regularly.
- Collaborate with clinical teams to support patient engagement and wellness outcomes.
Requirements:
- Diploma in Health Sciences (Health Management), Sports Science, or equivalent.
- At least 2 years of experience in healthcare, community care, or wellness program facilitation.
- Passionate about community service with a proactive and organized approach.
- Strong interpersonal and communication skills.
