About the Role
We are looking for a dedicated and people-centred individual to join our team in supporting the health and wellness journey of the individuals we serve. In this role, you will work closely with a multidisciplinary care team to deliver holistic, person-centred support that promotes better health outcomes and quality of life.
What You Will Do- Conduct health assessments and identify risk factors to guide personalised care planning
- Work collaboratively with individuals and their families to set meaningful, achievable health goals
- Provide education and guidance on lifestyle management, self-care practices, and available community resources
- Support individuals in building sustainable healthy habits and navigating personal health challenges
- Coordinate care activities and maintain accurate documentation using relevant IT systems
- Contribute to preventive health initiatives and support participation in national health programmes
- Build strong relationships with internal teams and external stakeholders to ensure continuity of care
- Leverage technology to improve health literacy and connect individuals to appropriate resources
- Min. Diploma in Health Promotion or Equivalent
- A genuine passion for community health and helping others
- Strong interpersonal and communication skills
- Comfortable working in a team-based, fast-paced care environment
- Organised, detail-oriented, and comfortable with documentation and IT systems
- Prior experience in a healthcare or community care setting is an advantage
