Occupational Therapist (Private Nursing Home) - Geylang

The Role

We are looking for a passionate and dedicated Occupational Therapist to join our team. In this role, you will play a key part in enhancing our residents’ quality of life through tailored therapy programs that promote independence and well-being.

• Conduct comprehensive assessments to identify residents’ functional abilities and rehabilitation needs.
• Develop, implement, and review individualized rehabilitation plans to support mobility, cognitive, and daily living goals.
• Carry out weekly reviews to evaluate residents’ progress and adjust therapy plans accordingly.
• Participate in multidisciplinary case discussions to ensure cohesive and coordinated care.
• Plan and execute discharge plans, ensuring a smooth transition and continuity of care post-discharge.
• Provide therapy interventions (individual and group sessions) focusing on functional, psychosocial, and adaptive skill development.
• Recommend and train residents or caregivers in the use of assistive devices and adaptive equipment.
• Maintain accurate documentation of therapy sessions, assessments, and progress reports in accordance with regulatory standards.
• Educate residents, families, and care staff on rehabilitation techniques, home programs, and strategies for independence.

• Bachelor’s Degree (or equivalent qualification) in Occupational Therapy.
• Compassionate, patient, and committed to improving residents’ quality of life.
