Perm Assistant Care Coordinator #HKN

Location: West

Working Hours: any 5 days a week, 2 shifts: 8am-5.30pm / 12.30pm-8.30pm

Salary: $2600 – $3300 (with shifts allowance)

Job Description:

• Conduct pre-admission screening for patients and caregivers
• Collaborate with nurses and clinicians to deliver holistic patient care at home, including monitoring patient condition and progress
• Identify and escalate care issues, deviations, or gaps to the appropriate healthcare team
• Manage enquiries and provide updates on care plans to patients and caregivers
• Participate in interdisciplinary meetings and contribute to care planning
• Maintain accurate documentation of patient progress and treatment plans
• Coordinate with community partners and healthcare providers to ensure continuity of care and referrals
• Develop care plans based on assessed patient needs
• Monitor vital signs via telehealth systems and escalate abnormalities
• Provide basic patient education and health coaching (e.g., nutrition, lifestyle changes, exercise)
• Ensure availability of care equipment and coordinate with vendors
• Support quality improvement initiatives, risk management activities, and other assigned duties

• Min. Diploma in a health-related field
• Good computer literacy and familiarity with standard software applications
• Willingness to work 2 shifts, including evenings, weekends, and public holidays
• Able to work independently in a fast-paced environment
• Strong interpersonal, communication, and teamwork skills
• Service-oriented with a positive and professional attitude
• Demonstrates initiative, integrity, and a commitment to quality care
• Keen to work in healthcare setting
