Bukit Batok - Care Coordinator (West / office hours) (ID:659847)

Responsibilities:

• Support the clinical team in advising clients on financial scheme applications and ensure timely updates for means testing and financial assistance.
• Assist in the onboarding process for new clients, including scheduling assessments, applying for subsidies, and addressing inquiries related to rehabilitation.
• Help assess client and caregiver needs, providing basic case management under the guidance of a case manager or social worker.
• Streamline access to community services for clients and caregivers by coordinating appropriate information and referral assistance.
• Participate in case discussions, conduct joint home visits with the multidisciplinary team, and prepare relevant monthly reports and statistics.

Requirements:

• Nitec/ Diploma holder
• Care coordinator / Social Work Assistant related experience will be an advantage
