Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Clinic Care Coordinator.

- Assisting in coordinating and providing patient care activities.
- To use of IT systems to track, perform and document established clinical indicators and care given to patient.
- Perform basic parameter measurements, close preventive screening and vaccination gaps in relation to national programmes/initiatives.
- Provide basic health education to improve patients’ knowledge on lifestyle management, social and physical activities through social prescribing
- Diploma in Health Promotion will be an advantage
- Prior healthcare experience will be advantageous
- Good verbal and written communication skills
- A team player with good interpersonal & communication skills
- Passionate to work in Healthcare industry
- Customer-oriented, a team player, a fast learner and serious with work
