- Evaluate, plan and implement care for chronic kidney disease patients
- Conduct teaching and counselling for patients and patient family
- Collaborate with Allied Health team and physicians in the delivery of patient care and service
- Maintain related database
- Provide patient's report to the Programme Manager or Doctor where required
- Any Ad hoc duties as per assigned
Requirement:
- Bachelor Degree in Life Science, Nursing, Medical Sciences or related medical fields
