Responsibilities:

• Conduct in-depth care needs assessments to help residents comprehend their health status and establish health-promoting objectives.
• Implement lifestyle interventions to modify and sustain behaviors that mitigate health risks and improve self-management of chronic conditions.
• Collaborate with the Community Health Team (CHT) to ensure effective service delivery and holistic care planning.
• Foster a supportive environment that motivates residents to take control of their health.
• Conduct home visits and be stationed at various community posts as required.

Requirements:

• Diploma in Sport Science, Sport Management, or Healthcare Service Management.
• Previous experience in a relevant field is preferred.
