Job description:
Role Overview:The Dietitian will provide holistic, evidence-based nutrition care to patients and caregivers in the primary care setting.
Services will be delivered face-to-face in clinic settings and/or via approved virtual care platforms with focus on chronic disease management. The role includes supporting and participating in health promotion initiatives aligned with national strategies such as EatWise and Healthier SG, contributing to continuous improvement in patient outcomes.
Key Responsibilities:
- Conduct comprehensive nutritional assessments and develop individualised, evidence-based nutrition care plans and patient education materials.
- Deliver high-quality clinical nutrition services to support disease management, prevention, and health maintenance in primary care.
- Maintain timely, accurate, and comprehensive clinical documentation in accordance with professional and organisational standards.
- Motivate and support patients in treatment adherence and sustainable lifestyle changes to optimise health outcomes.
- Contribute to the advancement of nutrition and dietetics practice through participation in clinical research, audits, and quality improvement initiatives.
- Support public education, health promotion programmes, and interprofessional training, including initiatives aligned with EatWise and HealthySG.
- Collaborate with multi-disciplinary healthcare teams to deliver support coordinated, patient-centred care and contribute to organisational objectives in clinical service delivery, patient education, and community outreach.
Job Requirement:
- Recognised Degree in Nutrition and Dietetics or equivalent
- Preferably 1-2 years of relevant clinical experience in healthcare settings with Dietetic services; fresh graduates are welcome to apply
- Preferably a registered dietitian with the Singapore Nutrition & Dietetics Association (SNDA) or any recognised Dietitians Association
- Resourceful and able to work independently and collaboratively across different polyclinics
- Demonstrated analytical, interpersonal and communication skills
- Proficient in written/spoken English; proficiency in local languages and/or dialects is an added advantage
