Job Title: Care Coordinator (Staff Nurse) – 3-Month Contract

Contract Duration:

Salary Range: $3,600 to $4,000 per month (negotiable)

Working Hours:

• Monday to Thursday: 8:30am – 6:00pm
• Friday: 8:30am – 5:30pm

• Participate in discharge planning and identify patients requiring post-discharge support.
• Provide follow-up care through phone calls and/or home visits to ensure care plans are understood and implemented.
• Assist patients and families in accessing and coordinating appropriate community services.
• Implement and evaluate timely care coordination interventions.
• Act as the liaison between patients, families, and the multidisciplinary team to achieve optimal patient outcomes.
• Monitor patients’ general medical condition during home visits and escalate concerns where necessary.
• Educate and support patients in adopting healthy lifestyle changes in the community.
• Assist in advance care planning and improving end-of-life care processes.
• Contribute to hospital quality improvement initiatives, research, and innovation projects.
• Support data collection and prepare reports/statistics for outcome tracking.

• Diploma or Degree in Nursing or equivalent professional qualifications.
• Minimum of 3–5 years of relevant experience in acute or community healthcare settings in Singapore.
• Prior experience in care coordination or case management is an advantage.
