Responsibilities:

• Ensure safe, competent, and individualized nursing care services.
• Uphold nursing standards in documentation, treatment, patient monitoring, and infection control.
• Allocate equipment to meet care requirements.
• Communicate and build rapport with patients and their families or caregivers.
• Coordinate patient care through advising physicians, provide direct patient care, evaluate conditions, and serve as the liaison between families and physicians.
• Facilitate and participate in Multidisciplinary Team meetings for smooth delivery of patient care and services.
• Observe legal requirements regarding storage and control of drugs and medical supplies.
• Ensure patient safety and report incidents to Nurse Manager / Nurse Clinician.
• Maintain proper documentation of patients’ condition and accurate collation of data.
• Monitor, reassess, and evaluate patients’ condition in implementing care plans and support family members to cope with advanced illness.
• Perform nursing care such as medication administration and wound dressing.
• Follow up to ensure continuity of patient care and treatment plans.
• Ensure proper use and care of equipment and economical use of consumables and supplies.
• Maintain confidentiality and discretion in matters involving patient care.
• Maintain a conducive and safe patient care environment.
• Mentor junior staff for performance excellence and career development.
• Enforce compliance with the Singapore Nursing Board's Standards of Practice for Nurses and Midwives, and the Code of Ethics and Professional Conduct.
• Stay informed of updated industry guidelines and develop new skills necessary for the job.
• Maintain high standards of housekeeping and workplace organization.
• Drive continuous improvements in own work area regularly.

Requirements:

• Registered with Singapore Nursing Board.
• Comfortable working shifts.
• Passion to serve and provide excellent customer service to various stakeholders.
• Willing to contribute as an effective team player.
• Able to perform under pressure.
• Post-registration experience in a hospital or hospice setting is an added advantage.
• Experience in Oncology, Palliative Care, Geriatrics, or Gerontology disciplines is an added advantage.
