What you'll be doing

Develop and implement individualised care plans tailored to residents’ needs
Monitor residents’ progress and adjust care strategies accordingly
Design and run programmes and activities that promote residents’ well-being
Prepare timely case documentation, including monthly interview records and biannual review reports

What we're looking for

Bachelor Degree in Social Work/ Psychology/ Counselling or related studies
1 years of similar working experience
Strong case management and documentation skills
Empathetic, organised, and able to work independently
