Bukit Batok - Healthcare Coordinator

Key Responsibilities:

• Build strong relationships with internal care teams, patients, and caregivers to ensure continuity of care.
• Coordinate and provide patient care activities, including measurement of basic health parameters.
• Support preventive health screenings and vaccination initiatives to close care gaps.
• Provide basic health education on lifestyle management, physical and social activities, and vaccination awareness.
• Leverage digital tools and health monitoring devices (e.g. home BP, glucose, weight) to enhance health literacy and self-management.
• Use IT systems to track, document, and report care activities and clinical indicators.
• Gather social determinants of health data to support better health and social outcomes.

Requirements:

• Diploma preferred
• Prior experience in healthcare or community care is preferred.
• Good verbal and written communication skills.
• Pleasant, team-oriented, and self-motivated with a passion for patient care.
• Able to communicate effectively in English and a local language/dialect.
• Proficient in Microsoft Office (Excel, Word, PowerPoint) and comfortable with digital tools and IT systems.
