Position Overview
The NUHS Virtual Care Centre (VCC) delivers patient-centred, technology-enabled care to patients in the community. VCC is a medical helpline that supports patients and community partners. Staffed by AH physicians, nurses, and care managers, the VCC provides timely medical advice based on assessment of symptoms and clinical queries.It supports patients with non-life-threatening symptoms or concerns.
As a Pharmacy Technician in the VCC team, you will play a key role in supporting the medication management needs of patients receiving virtual care. You will support tele-pharmacy sessions, coordinate medication fulfilment and assist with logistics for medication delivery.
Key Responsibilities
Virtual care support- Carry out counselling for newly initiated medication by VCC
- Address basic patient queries regarding medication handling, dosing schedules, supply status and escalating complex cases to the pharmacist
- Assist pharmacists in history taking and structured medication reconciliation for requests related to medication top-ups
- Identify and document findings before escalation to pharmacist for clinical review and approval of medication top up requests
- Process prescriptions for VCC patients
- Coordinate with pharmacist and procurement to support supply of non-formulary medications
- Facilitate timely home delivery of medications
- Work with pharmacy home delivery team and external vendors to track, confirm and troubleshoot deliveries.
- Escalate delivery failures or delays and coordinate prompt resolution with the care team.
- Participate in continuous quality improvement projects relating to VCC pharmacy workflow
- Support audits and ensure compliance with medication safety
- Possess Certified Pharmacy Technician Certification (CPTC) or Diploma in Pharmaceutical Science
- Minimum 1–2 years of hospital pharmacy experience preferred.
- Familiarity with pharmacy and hospital IT systems (e.g. EPIC).
- Strong coordination, communication, and interpersonal skills to work in a multidisciplinary care team.
- Highly organised and able to manage multiple responsibilities in a fast-paced, evolving care setting.
