Geylang - Medical Social Worker (Islandwide/Home Care/Office Hour) (ID: 679332)

Responsibilities:

• Conduct comprehensive psychosocial assessments of patients
• Coordinate care planning, ensuring a smooth transition from home to hospital or other care settings
• Identify and connect patients and families with community resources
• Provide urgent case intervention services
• Educate patients and families about healthcare systems and policies

Requirements:

• Degree in Social Work or Master's Degree in Social Work or equivalent
• Registered with Singapore Association of Social Work
• Experience in means testing and healthcare setting is advantageous
• Effective clinical skills in assessment, intervention, casework and counselling
• Excellent team player
