Senior Medical Social Worker (Hospice Care) #HZOT

Working Hours: Office hours

Job Scope:

• Conduct socio-economic assessments to support patients and families with financial and practical needs
• Provide ongoing counselling (individual/family) through face-to-face sessions and home visits
• Collaborate with community partners (educational, religious, social services) to support end-of-life care goals
• Support and prepare patients and families for end-of-life, including care planning and emotional readiness
• Participate in interdisciplinary care conferences with healthcare teams for holistic patient management
• Advise on financial assistance and discharge planning
• Perform fieldwork across hospitals and community agencies to address medico-social issues
• Prepare reports and present complex cases for discussion and review
• Manage complex casework and document all interventions within required timelines
• Provide clinical supervision and professional input when needed

Requirements:

• Minimum Degree in Social Work and / or Master in Counselling preferably
• Registered with the Singapore Association of Social Workers for Social Workers or Singapore Association for Counselling (SAC).
