Social Work Coordinator

Overview

Join a compassionate healthcare social work team supporting patients and families through financial assistance, discharge planning, and care coordination. This role provides operational and administrative support while working closely with Medical Social Workers to enhance patient care and service experience.

• Support the daily administrative and operational functions of the department, including frontline counter duties.
• Conduct intake interviews, financial assessments, and assist patients with financial assistance applications.
• Support discharge planning and coordinate referrals with internal and external care providers.
• Assist Medical Social Workers with case management, patient follow-ups, and care coordination.
• Provide translation assistance and patient home visit support where required.
• Liaise with multidisciplinary teams and community partners to facilitate seamless patient care.
• Participate in process improvement and quality improvement initiatives.
• Support department programmes, projects, and administrative activities.

• Diploma in Social Work or related discipline.
• Prior experience in healthcare, social services, or community care settings will be advantageous.
• Good interpersonal, communication, and problem-solving skills.
• Compassionate and service-oriented with the ability to work with patients and caregivers from diverse backgrounds.
• Proficient in Microsoft Office and basic IT systems.
• Able to work independently as well as collaboratively in a multidisciplinary environment.
