Community Engagement Associate (Active Aging)
You will take ownership of a specific community cluster, supporting outreach, programme coordination, and stakeholder engagement.
Responsibilities- Organise and coordinate community engagement programmes tailored to elders' needs, including befriending efforts and community screenings.
- Conduct assessments to understand individual elder needs and develop personalised engagement plans, including home visits, care planning, and service referrals.
- Prepare promotional materials and collaterals to publicise programmes and services. Assist in AAC membership recruitment, publicity, registration, and retention.
- Support the evaluation and review of programmes for effectiveness and improvement. Ensure all activities comply with MOH’s AAC Service Model requirements.
- Ensure safety procedures are followed during all programmes. Maintain hygiene and environmental safety of activity areas and ensure equipment is in good working condition.
- Attend to enquiries and service requests from the community. Monitor the Alert Alarm System (AAS) during office hours and provide necessary assistance.
- Recruit, manage, and maintain a database of volunteers. Liaise with internal and external partners to implement programmes and services.
- Assist with programme administration, operations, and logistics. Maintain AAC management system records and prepare reports and claims for stakeholders.
Job Requirements:
- Minimum Diploma in relevant disciplines such as Social Work, Psychology, Gerontology, etc.
- Prior experience in Senior Care Centre, Active Ageing Centre or similar setting would be Advantageous.
- Passionate about serving elders.
- Resourceful, proactive, responsible, and focused on clients.
- Team player with strong communications, interpersonal and organisational abilities.
- Capable of working independently and under time constraints.
- Proficient in the use of Microsoft Office Applications.
