- Salary: $2,350 – $3,000 per month
- Full-Time, Able to Commit Working half-day on Saturdays

Key Responsibilities:
- Support and coordinate patient care activities alongside healthcare professionals.
- Engage patients and caregivers to promote preventive health screenings and vaccinations.
- Use IT systems to track clinical indicators and document care.
- Educate patients on lifestyle management, health monitoring devices, and social prescriptions.
- Gather and report social determinants of health that may impact care outcomes.
What We’re Looking For:
- Diploma preferred; qualifications in health promotion are an advantage.
- Some healthcare experience preferred.
- Strong interpersonal and communication skills in English and a local language/dialect.
- Proficient in Microsoft Office and IT-savvy.
- Positive, self-motivated, and a strong team player with a heart for patient care.
