Social Work/ Care Coordinator (Public Healthcare) - Geylang

Job Description:

• Assist Medical Social Worker/ MSW team in casework, programs and administrative support
• Work closely with patient, including intake interviews, financial assessment, initial family assessment for care arrangement, home visits.
• Provide support to patients and their family members and handle their enquiry at the reception counter or through enquiry lines
• Handle Medifund application and submission.
• Prepare and submit monthly or quarterly.
• Monitor and keep track of department funds.

Requirements:

• Diploma in Social Work, Health Promotion, or related field
• Prior experience in the healthcare setting is an added advantage
• Strong interpersonal skills, a positive attitude, and a team-oriented mindset
