Senior/Staff Nurse, Community Nursing
The role is responsible to work collaboratively with the interdisciplinary care team and community care partners to provide holistic care and support to clients in the community. To provide accurate and timely documentation of patient assessment, care plan, interventions rendered and evaluation.Able to refer/escalate to appropriate healthcare providers / community care partners according to patient's needs based on pre-defined protocols or guidelines.
Job Requirements:
- Diploma / Degree in Nursing
- At least 2 years of relevant working experience in the related field
- Registered with the Singapore Nursing Board
- Excellent interpersonal and communication skills
- Proficient in Microsoft Office applications
- Proactive team player
- Resourceful and able to work independently with minimum supervision
- Able to manage multiple tasks simultaneously and remain flexible in a changing environment
