Medical Social Worker (COMIT) - Bukit Batok

Responsibilities and Duties:

Requirements:

• Degree / postgraduate diploma in Social Work.
• Must have an interest to work with the adults above 40 years old and the elderly population in the community setting.
• Experienced in Community / Hospital setting for at least 2-3 years.
• Good team playerand possess good interpersonal, communication, writing & IT skills.
• Familiar with the healthcare environment will be an added advantage.
• Ability to communicate with vernacular languages speaking clients.
• Ability to multi-task, work independently and network with other service providers.
• Candidates with dementia or mental health training or working experience will be an added advantage.
• Has experience using psychotherapy with mental health patients.

Specific:

• Conduct assessment and provide intervention and support to clients with mental health issues and/or dementia, eg. psychoeducation, case management and counselling, as well as caregiver support and training.
• Plan, implement and review services and activities to meet the needs of the clients and/or caregivers.
• Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance to the COMIT programme requirement.
• Keep and maintain client and programme data and documents, in accordance to the COMIT programme requirement, as well as the prevailing personal data protection and security guidelines.
• Support in the development of COMIT service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).

Job Requirements:

• Social Work trained, preferably with 2 to 3 years’ social work experience. The Medical Social Worker is to conduct mental health assessment, provide home based direct intervention, support to clients and/or caregivers with mental health issues, and/or dementia in accordance to the Community Intervention Team (COMIT) programme requirement. For instance: psychoeducation, case management and counselling, as well as caregiver support and training.
