As a Medical Social Worker (Various Settings), you will be responsible for the following duties:

• Assess the psychological, emotional, social, economic, and physical impact of a patient's medical condition on their lives.
• Provide counselling, crisis intervention, and emotional support to patients and their families.
• Help patients and families access and utilize community resources, such as financial assistance, housing, transportation, and healthcare services.
• Collaborate with other healthcare professionals to develop treatment plans that address the patient's holistic needs.
• Provide support and resources for caregivers, recognizing the impact of illness on families.
• Assist with discharge planning, ensuring a safe and smooth transition back home or to another care setting.
• Maintain accurate and confidential records of their interactions with patients and families.

Requirements:

• Min Degree in Social Work/Post Graduate Diploma in Social Work or equivalent.
