As a Care Coordinator (Healthcare), you will be responsible for the following duties:
- Work with a team of doctors and nurses to take care of a group of patients with chronic conditions.
- Screen patients for relevant health issues.
- Guide patients to understand the importance of annual eye, foot and health screenings.
- Assist and educate patients about self-care, performing vision testing and basic assessments.
- Coordinate the care for patients with the healthcare team.
- Any other ad hoc duties as assigned.
- Possess a Diploma in Health Management & Promotion or equivalent.
