As an Case Management Officer, you will be responsible for the following duties:

• Coordinate and support multidisciplinary care for stem cell transplant patients, including donor searches and referrals.
• Arrange and track patient and donor appointments, investigations, and required clinical documentation.
• Act as a liaison between patients, families, and stakeholders to ensure smooth care transitions.
• Educate patients and families on the stem cell transplantation care journey.
• Conduct follow-ups and assist patients in navigating services and locations.
• Handle administrative duties such as audits, documentation, database maintenance, and meeting secretariat tasks.
• Contribute to quality improvement initiatives and identify process enhancements.
• Provide cross-coverage for the Clinical Research Coordinator and take on other tasks as assigned.

• Degree in Nursing or Allied Health or Science related.
