Nurse Counsellor (Primary Care Network)

We are looking for a compassionate and patient-centric Nurse Counsellor to join our Primary Care Network (PCN) team.

Key Responsibilities

• Provide health education and counselling to patients with chronic conditions such as diabetes, hypertension, and high cholesterol
• Guide patients on lifestyle modification, medication adherence, and preventive healthcare
• Support patients in understanding their treatment plans and follow-up requirements
• Build rapport and maintain ongoing engagement with patients

• Coordinate patient care plans with doctors and clinic teams
• Conduct patient follow-up calls and appointment coordination
• Monitor patients enrolled under PCN programs
• Escalate patient concerns or abnormal findings appropriately

• Perform basic nursing assessments and screening support where required
• Maintain accurate patient documentation and records
• Support clinic operations and program reporting
• Ensure compliance with MOH and clinic guidelines

• Diploma or Degree in Nursing recognised in Singapore
• Registered with Singapore Nursing Board (SNB)
• Experience in primary care, chronic disease management, community care, or patient counselling will be an advantage
• Good communication and interpersonal skills
• Comfortable engaging patients from diverse backgrounds
• Positive attitude and team player
