Senior Medical Social Worker - Custer Lead
About the role
We are seeking a highly experienced Senior Medical Social Worker to lead our Custer team at ECON Healthcare (Asia) Pte Ltd, a leading healthcare provider in the Redhill Central Region. As the Senior Custer Lead, you will oversee a team of social workers and play a crucial role in developing and implementing comprehensive care plans for our patients.This is a full-time position that offers excellent opportunities for professional growth and development.
What you'll be doing- Providing expert-level clinical social work services to patients and their families, including assessment, counselling, and discharge planning
- Leading and managing a team of social workers, ensuring high-quality service delivery and supporting their professional development
- Collaborating with multidisciplinary teams to develop and implement holistic care plans that address the social, emotional, and practical needs of patients
- Advocating for patient rights and ensuring compliance with relevant laws, regulations, and organisational policies
- Maintaining accurate and up-to-date documentation, including case notes and progress reports
- Participating in research, quality improvement initiatives, and other projects to enhance the delivery of social work services
- A Master's degree in Social Work or a related field, with a current registration as a Social Worker in Singapore
- Minimum 5 years of experience as a medical social worker, with a proven track record of providing exceptional care to patients and their families
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with healthcare professionals and community partners
- Strong leadership and mentoring skills, with the ability to manage and develop a team of social workers
- Proficient in case management, crisis intervention, and discharge planning
- Demonstrable experience in program development, implementation, and evaluation
- Commitment to ethical practice and a strong understanding of relevant laws, regulations, and professional standards
