About the job - Medical Social Work Associate (Nursing Home)

To support the delivery of psychosocial care and case management services for our residents and their caregivers. You will assist the Medical Social Worker in conducting assessments, facilitating care coordination, and implementing programmes that enhance the well-being and dignity of our elders.

Responsibilities:

  • Support the Medical Social Worker in gathering social and background information to facilitate bio-psycho-social-spiritual (BPSS) assessments and care planning.
  • Assist in monitoring residents' progress, coordinating care activities, and following up on action plans under the guidance of the Medical Social Worker.
  • Assist in organising and running psychosocial or therapeutic programmes and activities that promote social connection and emotional well-being.
  • Maintain case documentation, prepare reports, and support financial assistance applications such as Medifund or Subsidy Deviations.
  • Support pre-admission and discharge processes, including follow-up with caregivers and coordination with relevant agencies.
  • Assist in facilitating Advance Care Planning (ACP) discussions and provide practical and emotional support during grief and bereavement situations under supervision.
  • Participate in audits, data collection, and initiatives to enhance the quality and efficiency of social work services.
  • Work closely with nurses, therapists, and other allied health professionals to ensure holistic care for residents and their caregivers.

Requirements:

  • Diploma in Social Work, Social Services, Counselling, or a related discipline from a recognised institution.
  • Prior experience in a healthcare, community, or eldercare setting will be an advantage.
  • Strong interpersonal and communication skills with the ability to engage residents, caregivers, and colleagues effectively.
  • Empathetic, patient, and able to manage sensitive situations with professionalism and care.
  • Proficient in Microsoft Office applications and able to maintain proper case documentation.
  • A team player with good organisational skills and a proactive attitude towards learning and service improvement.
