Medical Social Worker - Toa Payoh

Responsibilities

Identity presenting / underlying challenges and determine appropriate Advance Care Plan to enhance, maintain and promote psychosocial functioning of patients.
Provide emotional support for End-of-Life Care, Critical Care, Grief and Bereavement to patients and their families.
Lead patients and caregivers support group programs.
Collaborate with other community service providers to provide appropriate services for continuity of care when patients are discharged.
Maintain proper compliance documentation for case management and audit purposes.

Requirements

Bachelor degree/Master in Social Work & or Social Science discipline.
Minimum 3 years relevant experience, with specialization in Counselling and Psychotherapy.
Registered with Singapore Association of Social Work & Registered Counsellor.
Good clinical experience and Certified CTRT practitioners will be an added advantage.
Passionate about serving elders.
Resourceful, proactive, responsible, and team player with strong interpersonal, communication skills and patient-oriented personality.
Mature and confident in dealing with patients, NOK and the public.
