As a Care Coordinator, you will be responsible for the following duties:
- Assist in referring GP patients to healthcare, community, and social service providers, including arranging & following up on appointments.
- Work closely with nurses to identify patients who may require counselling and additional support services.
- Liaise with service providers to coordinate deployments, process payments, and ensure smooth service operations.
- Collaborate with clinic assistants to encourage patients to attend scheduled chronic care appointments, blood tests, and health screenings.
- Compile and maintain clinical data for analysis, and support the tracking and reporting of clinical indicators to stakeholders.
- Travel to assigned GP clinics within the region for average 2 days per week.
Requirements:
- Min Diploma in Health Management or equivalent field.
