As a Care Coordinator, you will be responsible for the following duties:
- Empower patients and caregivers through tech and education.
- Build strong ties with care teams and the community.
- Support preventive screenings and vaccinations.
- Use IT systems to track care and outcomes.
- Help reduce hospital readmissions and escalations.
- Address social determinants of health.
- Conduct basic health checks and promote home monitoring.
- Provide health advice and social prescribing.
Requirements:
- Possess a Diploma in Health Management & Promotion or equivalent.
