The jobholder supports social work responsibilities of the centre, linking clients up with community resources and facilitating admission of clients to CBC services.
- Conduct Information and Referral service to help meet the needs of home-bound clients, and home visits where necessary
- Go through the service agreement with clients and their caregivers to ensure service scope is able to meet client’s needs and expectation. To explore alternatives with clients and their caregivers if service scope is unable to meet client’s needs and expectation.
- Respond to complaints/feedback from clients and their caregivers. Take follow-up actions as necessary.
- Complete documentation in a timely and accurate manner and manage clients’ case files, ensuring prompt updates to individualise care plans, involvement of family and discharge summaries.
- Perform Financial Assessment and assist needy client with Financial Assistance application.
- Demonstrate and maintain good interpersonal skills with clients, Staff and public at all times to build co-operation and trust.
- Conduct himself/herself professionally in his/her work and communication with others.
- Work with community partners to craft holistic care plans or transitions for clients.
- Perform other duties as assigned by Centre Manager.
